Hello. I am 25(f) and I have been having this issue ever since my fiancé and I got together. I have slept with about 25-30 other guys before I met my SO and this has never been an issue, but every single time my SO and I have sex, my perineum tears. I have HSV-1 but I only get breakouts on my gums, but that may be the cause. I also have been on birth control for 12 years. We try to do more foreplay but it still ends up tearing. It’s a constant cycle of it tearing, us waiting for it to heal so no sex for 1-2 weeks and then it tearing again. We’ve tried slow sex and it tears less but we’ve never found a permanent solution. He likes rough sex and I used to as well. I miss just being able to have normal sex again. I have asked two doctors and they both said my vagina seems normal and just to do extensive foreplay and use lube. It tears anyways.
