Why do corporations face fines instead of managers, leaders, or employees within a corporation being held directly accountable? /u/DesertDwellingWeirdo No such thing as stupid questions

I’m curious at what level of the legal system this begins in. For example, is it the fault of prosecutors for turning away the case, the investigators for refusing to dig deeper and determine a specific responsible person, or the judge for handing down a fine instead of a more severe penalty?
