28M with a history of migraines

In 2019, there was a period where I was unable to masturbate for a long time. When I finally could again, I found myself getting some pretty bad headaches when I got close to orgasm. One headache which I got while masturbating standing up in the shower was the worst pain in my life and remained for two days. Irresponsibly as we were traveling, we visited a paramedic while picking up a rental car from the airport who said I was probably dehydrated. I refrained from masturbating for a few days, when I started again, everything was fine. Nothing ever came of it.

Now, starting about a week ago. The headaches have come back. There have been no major changes in my masturbation habits, but I am now having frequent sex with a new partner. At first I was anxious and struggling to get erect, now my erections are fine. The headaches are nowhere near as intense as in 2019, its pain usually always on one side of my head and while it sometimes throbs more painful with orgasm, it often emerges more while getting aroused before actually cumming as well. Sometimes I take an ibuprofen before sex, other times I don’t, the pain while quite annoying has generally not been bad enough to stop me from wanting to have sex. Possibly because me and my new partner were long distance and are quite excited to finally be intimate.

But yes, ibuprofen before or after significantly lessens them and within an hour or so they are usually gone. Fresh air for whatever reason seems to cause immediate improvements as well. If we have sex standing up, its significantly worse than lying down. I did get over Covid recently if that might have an effect? Anyhow, I would say generally the headaches have gotten a little better, but we’ve also been more conscientious the last couple days about the positions we are having sex.
