If a medical procedure is deemed “not medically necessary” then why is the patient on the hook for the bill? /u/veryblocky No such thing as stupid questions

This is something I’ve never understood, insurance can deny a claim if the procedure wasn’t medically necessary. But, how should a layman be able to know that before consenting to it? If it was medically unnecessary, then the patient certainly shouldn’t have to pay for it, because the hospital did it unnecessarily.

Shouldn’t it be on the medical staff to know if something is medically necessary, and if the insurance disagrees then the hospital should have to show that it was.

It feels wrong that the patient should be left with the bill, when they’re not in a position to determine what’s needed
